—— WHAT IS AWAKE SPINE SURGERY?

Spine surgery without going to sleep.

Awake spine surgery — also called conscious spine surgery or awake endoscopic spine surgery — uses spinal or epidural anaesthesia to numb just the surgical area. You remain relaxed, aware, and comfortable on the table, but feel nothing below the injection point.

When combined with full endoscopic technique — a 7 mm portal, HD camera, and specialised instruments — the surgeon can decompress a disc, remove a fragment, or widen a narrowed foramen through the smallest possible access, without the risks of general anaesthesia.

Traditional spine surgery (general anaesthesia)

  • Whole-body anaesthesia with breathing tube (intubation)
  • Higher risk for elderly, diabetic, cardiac, or obese patients
  • Post-anaesthesia confusion (POCD) in some patients over 60
  • Longer recovery — 3–4 hrs before alertness returns
  • Overnight admission usually required

Awake spine surgery (spinal/epidural anaesthesia)

  • Regional block — you breathe naturally throughout
  • Dramatically safer for high-risk and elderly patients
  • No post-anaesthesia confusion; alert within minutes
  • Walk and eat within 2 hours of theatre
  • Same-day discharge in 98% of cases

—— WHO BENEFITS MOST

Awake surgery is especially recommended for:

Elderly patients (60+)

Post-operative cognitive decline (POCD) after general anaesthesia is a well-documented risk in older adults. Awake endoscopic surgery eliminates this risk entirely.

Diabetic patients

Blood sugar swings under general anaesthesia increase infection and healing risk. Regional anaesthesia provides a more controlled metabolic environment.

Cardiac and lung conditions

Patients with hypertension, heart disease, COPD, or sleep apnoea told they are high risk for general anaesthesia are ideal candidates for the awake pathway.

Obese patients

Airway management is more challenging with high BMI. The awake endoscopic pathway avoids intubation and reduces post-op complications significantly.

Previously refused surgery

If another surgeon told you that you are too high risk for spine surgery — awake endoscopic surgery may change the equation entirely.

Anxiety about general anaesthesia

Many patients have a strong personal preference to avoid general anaesthesia. Awake surgery respects that preference while delivering the same outcome.

—— HOW THE PROCEDURE WORKS

Step by step — what happens in theatre.

Step 1 — Spinal / epidural block

The anaesthesiologist administers a spinal injection or epidural catheter. The lower body becomes numb within minutes. Light sedation keeps you relaxed.

Step 2 — 7 mm portal placement

Dr. Sayuj makes a 7 mm incision and guides a tubular trocar to the disc level under fluoroscopic guidance. No muscle is cut.

Step 3 — Endoscopic decompression

An HD endoscope is passed through the trocar. The herniated fragment, bone spur, or thickened ligament is removed under direct vision.

Step 4 — Real-time nerve check

Because you are awake, Dr. Sayuj can ask in real time whether leg pain has resolved. This live feedback is impossible under general anaesthesia.

Step 5 — Closure

The trocar is removed. The 7 mm incision is closed with a single stitch. Total theatre time: 60–90 minutes for most disc cases.

Step 6 — Recovery and discharge

The block wears off in 2–3 hours. You walk, eat, and go home the same day with a prescription and a 48-hour follow-up call.

—— PATIENT STORY

“My cardiologist said I was too risky for surgery. Dr. Sayuj did it under spinal block. I walked out the same evening. My leg pain of two years — gone.”

—— FREQUENTLY ASKED QUESTIONS

Awake spine surgery — what patients ask most.

Will I feel pain during the surgery?

No. The spinal or epidural block completely numbs the surgical region. You may feel mild pressure but no pain. Light sedation ensures you remain calm throughout.

Is awake spine surgery safe?

Yes — and for high-risk patients it is often safer than general anaesthesia. Over 1,000 cases at our centre have demonstrated a complication rate below 1.5%.

Is awake spine surgery available in Hyderabad?

Yes. Dr. Sayuj Krishnan performs awake full endoscopic spine surgery at Yashoda Hospital, Malakpet, Hyderabad — one of the few centres in India dedicated to this pathway. Patients travel from across Telangana, Andhra Pradesh, Maharashtra, and internationally.

How is recovery different from open spine surgery?

Dramatically faster. Most patients walk within 2 hours of theatre and return to desk work in 1–2 weeks. Open surgery requires 2–4 days in hospital and 4–6 weeks off work.

What is the cost of awake spine surgery in Hyderabad?

As a daycare spine surgery centre, total cost is significantly lower than traditional inpatient surgery — no ICU stay, shorter admission, faster return to work. Book a consultation with your MRI for a personalised estimate.
